Once your appointment has been scheduled, we recommend you follow the below steps:
Download and complete the new patient paperwork. If you prefer, you can also contact the office to email it to you.
Take the following items to your first appointment:
- Current medications
- List of questions
Appointment Checklist
To make your appointment successful, we recommend that you bring:
- Completed paperwork (or email ahead of appointment)
- Insurance Card
- Photo ID
- List of current medications
Accepted Health Insurance Plans
We accept most HMO, Medicare HMO, POS, PPO, and EPO plans, including straight Medicare. To find out if we accept your insurance plan, call our office at (650)570-5861
Copayments & Billing
Most insurance plans require a copayment, which is due at the time of service. After your appointment, your insurance will be processed, and you will be billed for any services that are not covered. If you are uncertain about whether a particular service is covered, please call your insurance provider directly. If you have questions about a bill, please call the phone number listed on the billing statement.
